Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded by Pfizer Inc. and the National Comprehensive Cancer Network® (NCCN®) to support patients with metastatic breast cancer (mBC). The grant will enable the deployment of Avident Health's MyNavigator™ digital platform, a home-based tool designed to bridge communication gaps between clinic visits, according to the announcement.
The initiative aims to address persistent challenges in metastatic breast cancer care, where complex treatment regimens require ongoing adherence, symptom monitoring, and whole-person support. Patients and their families often struggle to navigate clinical guidelines and access community resources outside of traditional appointments. MyNavigator seeks to fill that void by delivering personalized, patient-friendly education aligned with NCCN Guidelines for Patients®, facilitating enrollment in Unite for HER's free supportive services, and equipping informal family caregivers to participate in shared decision-making.
Specifically, the platform translates complex clinical pathways into accessible, bite-sized modules tailored to each patient's treatment journey, empowering them to engage actively in informed decisions. It also integrates Unite for HER's comprehensive services—including professional nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—directly into daily navigation prompts. Unique to this project is the inclusion of family members and daily care partners in a secure communication loop, ensuring they have the context and early symptom alerts needed to support treatment adherence at home. Additionally, patient-reported check-ins and symptom trends are captured to alert clinical teams early, potentially preventing complications and reducing acute care utilization.
The collaboration brings together three distinct areas of expertise. Avident Health, a digital health company focused on cancer care coordination, provides the MyNavigator platform. Unite for HER, a national nonprofit, offers integrative therapies and supportive services to breast and ovarian cancer patients at no cost. NCCN, a not-for-profit alliance of leading cancer centers, contributes its gold-standard clinical guidelines. Pfizer funds the grant as part of its commitment to improving cancer care.
"Navigating metastatic breast cancer is a daily reality that extends far beyond the four walls of the clinic," said Dr. Maen Farha of Avident Health. "By combining the proactive digital coordination of MyNavigator with Unite for HER's exceptional integrative services and NCCN's gold-standard guidelines, we are creating a continuous circle of support that empowers patients, supports clinical teams, and actively engages the family caregivers who are vital to treatment success." Sue Weldon, Founder and CEO of Unite for HER, added that the collaboration enables the organization to reach patients and families directly at the moment they need guidance, removing barriers to integrative nutrition, wellness, and emotional support resources.
The grant-funded project will measure key quality improvement endpoints over its duration, including improvements in patient knowledge concordance with NCCN Guidelines, rates of supportive service utilization, patient-reported decisional conflict, and therapy adherence metrics. These outcomes will help determine the effectiveness of the digital navigation approach in real-world settings.
For patients and caregivers, this initiative could mean easier access to educational resources and supportive services that are often fragmented or difficult to find. For the healthcare industry, it represents a growing trend toward digital tools that extend care beyond the clinic and integrate caregiver support, potentially reducing complications and acute care use. As metastatic breast cancer remains a complex, lifelong condition, such collaborations may set a precedent for how technology, nonprofits, and clinical guidelines can work together to improve quality of life and treatment outcomes.
